Upgrading Health Assessments with ConXus

Since 2015, this health plan has trusted PDHI to provide certified health assessments to their 300,000 Medicare, Medicaid, and commercial members across the northern U.S.

PDHI’s NCQA WHP 5 certified Medicaid questionnaires for adults and children were customized to accommodate regulatory requirements.

By combining the ConXus Platform with PDHI’s Completion Support services to deliver personalized multi-touch and multi-channel outreach, the plan was able to reach over 50% completion for its Medicare members.
